- Bus Registration (4K-12 only) — only if riding the bus to and/or from school
- Children of Dual-Household — only if you are a dual-household family and you have not filled this out yet

- Gift Card Agreement and Delivery Waiver
- Google Account Consent and Technology Acceptable Use (4K-12 only)

- Alternate Year Physical Card (6-12) — only if participating in athletics and submitted a complete physical form last year
- Athletic Waivers (6-12) — only if participating in athletics
- Complete Physical Form
(6-12) — only if participating in athletics and did not submit a complete physical form last year - Virtual Learning Device (4K-12)
One Per Family Optional
- Automatic Lunch-After Care-Fees Registration — only if you wish to have budgeted amounts automatically paid each month for lunch and extended care
- Parent Association Dues
