2023-24 Athletic form (Grades 6-12)
- If your middle/high school student will be playing sports during the 2023-24 school year our Athletic Form must be filled out. The following athletic form includes
- WIAA Sports Physical Form that must be filled out by your doctor (must be done every other year)

- Middle School - $75
- High School - $100
