Absence notification Forms
When to use these forms:
- When your student is sick. Please report any illnesses ASAP, the night before or preferably before (but no later than) 7:30am.
- To report any future absences such as doctor appointments, vacation, etc. This also includes any driver's education classes. Please complete the form one week in advance.
