Who should use this form?
This form should be completed by anyone who donates an item, product, or service to Columbus Catholic Schools (CCS) or one of our clubs, organizations, athletic teams, or activities. Completing this form ensures that we can provide you with an official thank-you letter for your records, which you may use for tax purposes.
Important for Fundraisers
If your club, organization, athletic team, or activity is hosting a fundraiser, it is your responsibility to ensure that every donor who contributes an item, product, or service receives this form.
