Anybody who donates an item/product/service to CCS or one of our clubs/organizations/athletic teams/etc. so we can send you a thank you letter to use for tax purposes.
If your club/organization/athletic team/etc. is holding a fundraiser it is your responsibility to ensure that any donor who donates an item/product/service receives this form.
