Athletic Forms

If your middle/high school student will be playing sports during the school year, our Athletic Form must be filled out. The following athletic form includes:

• WIAA Sports Physical Form must be filled out by your doctor (must be done every other year)
• Student & parent information and emergency contacts
• Athletic Fee: Middle School - $100; High School - $125
• Medical information
• Concussion Information — Parent & Athlete Agreement
• Statement of Acknowledging Receipt AND Student Athlete Responsibility
• Concussion Experience, Symptoms
• Sudden Cardiac Arrest Parent & Athlete Agreement
• Handbook Read Verification and Code of Conduct Agreement
• Insurance Waiver
